VA State School Entrance Form (signed by pediatrician)
Per Fairfax County regulations, all drugs or medications to be administered to students require a form signed by a doctor. This includes, but is not limited to, EpiPens, Benadryl, bug spray, sunscreen, diaper cream, any prescription or over the counter drugs or medication. We strongly encourage families to apply creams and sprays before school as needed.
